Abstract:Objective To synthesize the best evidence regarding transition from pediatrics to adult care for adolescents with chronic kidney disease (CKD), and to provide references for developing transition programs and optimizing transition care. Methods A systematic search was conducted across multiple databases for literature from database inception to January 2025, including clinical decision support report, guideline, systematic review, evidence summary, expert consensus report, etc., related to transition management of adolescents with CKD to adult care. Two researchers independently evaluated methodological quality of the included literature and extracted data.Results A total of 14 publications met the inclusion criteria, including 4 clinical decision support reports, 3 guidelines, 2 expert consensus reports, 4 systematic reviews, and 1 randomized controlled trial.Twenty pieces of evidence were summarized across 5 themes:formulating transition plan, multi-party collaboration and coordination, transition support, transfer to adult care, and periodic audit and quality improvement. Conclusion The synthesized evidence offer an evidence-based foundation for designing structured transition interventions, thereby facilitating smooth transition, ensuring continuity of care and reducing the burden on both families and society.
