    摘要:

    目的 了解下肢慢性伤口伴水肿患者生活质量现状及其影响因素,为制订个性化护理干预措施提供依据。方法 2023年11月至2024年6月,采用便利抽样法选取4所三级甲等医院收治的214例下肢慢性伤口伴水肿患者,使用一般资料调查表、慢性伤口生活质量问卷和伤口积极心理状况量表进行调查。结果 下肢慢性伤口伴水肿患者的生活质量总分为(59.07±9.57)分;生活质量总分及各维度得分与积极心理状况得分呈负相关(均P<0.05);多元线性回归分析显示,年龄、职业状况、并存糖尿病、PUSH评分、水肿程度、积极心理状况是影响下肢慢性伤口伴水肿患者生活质量的主要因素(均P<0.05)。结论 下肢慢性伤口伴水肿患者生活质量处于极差水平,医护人员应重视老年群体及患者的社会经济状况,采用多学科管理模式加强并存疾病、创面及水肿的评估与管理,制订个性化干预措施和消肿策略,促进伤口愈合,同时关注患者的心理状况,给予社会支持和心理干预,提高患者生活质量。

    Abstract:

    Objective To understand the current status of quality of life and its influencing factors in patients with chronic leg wounds accompanied by edema, so as to provide a reference for developing personalized intervention strategies.Methods From November 2023 to June 2024, 214 patients with chronic leg wounds accompanied by edema admitted to four tertiary hospitals were selected by convenience sampling, then they were investigated by using a general information questionnaire, the Questionnaire on Quality of Life with Chronic Wounds (Wound-QoL), and the Well-Being in Wounds Inventory (WOWI).Results The participants′ total score of the Wound-QoL was (59.07±9.57) points, its total score and each subscale scores were negatively correlated with WOWI score (all P< 0.05).Multiple linear regression analysis showed that, age, occupational status, comorbid diabetes mellitus, PUSH score, degree of edema, and WOWI were the major influencing factors of quality of life in patients with chronic leg wounds accompanied by edema (all P<0.05).Conclusion The quality of life in patients with chronic leg wounds accompanied by edema is extremely poor.Medical staff should pay attention to the elderly population and patients′ socioeconomic status, adopt a multidisciplinary management approach to strengthen the assessment and management of comorbidities, wounds, and edema. Personalized interventions and decongestion strategies should be developed to promote wound healing, and patients′ mental state should be paid attention, social support and psychological interventions should be provided to enhance their quality of life.
