Objective To summarize the best evidence on the management of transition preparedness for adolescents with HIV/AIDS aged 10-24 years from pediatric to adult healthcare,so as to provide references for policymakers,clinical healthcare providers and caregivers. Methods The PIPOST model was used to construct the evidence-based question. The literature on transition preparedness management for adolescents with HIV/AIDS was retrieved according to the "6S" evidence pyramid model,from database inception until April 2024. The selected literature was evaluated for quality, evidence items were integrated, and the evidence level was assessed. Results A total of 17 articles were included. Totally 36 evidence items were summarized from five dimensions and thirteen categories:transition preparedness principles (patient-centered,family involvement, multidisciplinary support), community resource integration (friendly services,medical insurance, career development), healthcare system coordination (information sharing,continuous services, transition framework), individual development support (self-management, psychological care), and dynamic transition implementation (shared decision-making, ongoing assessment). Conclusion The evidence summarized in this study on the transition preparedness management for adolescents with HIV/AIDS is comprehensive,scientific,and applicable,which helps improve the quality of healthcare services and health outcomes during the transition period for adolescents with HIV/AIDS.
