    摘要:

    目的 观察不同灸温对压力性损伤大鼠创面修复的影响。方法 选择65只SPF级SD雄性大鼠,取其中60只根据缺血-再灌注原理进行压力性损伤造模,在成功造模的基础上将模型大鼠随机分为模型组、(42±1)℃艾灸组、(45±1)℃艾灸组、(48±1)℃艾灸组,每组15只。模型组创面仅给以碘伏消毒,1次/d,干预12 d;艾灸组用直径18 mm的艾条,施以回旋灸,10 min/次,1次/d,干预12 d。分别于干预第 3、7、10天观察比较四组大鼠创面愈合情况、愈合时间及愈合率、病理改变,并观察血管内皮细胞生长因子A(VEGF-A),缺氧诱导因子1α(HIF-1α),磷酸化蛋白质丝氨酸苏/氨酸激酶(p-Akt)的表达水平。结果 干预第3、7、10 天,与模型组比较,艾灸组创面愈合时间显著缩短;(45±1)℃艾灸组创面愈合时间短于(42±1)℃艾灸组及(48±1)℃艾灸组;创面愈合率显著高于(42±1)℃艾灸组及(48±1)℃艾灸组(均P<0.05)。HE染色结果显示,(45±1)℃艾灸组炎症细胞少于其他组,新生的肉芽组织多于其他组,血管内皮细胞及成纤维细胞排列更紧密。免疫组化结果显示,干预第3、7天,与模型组比较,艾灸组VEGF-A、HIF-1α及p-Akt表达水平均较高;与(42±1)℃艾灸组及(48±1)℃艾灸组相比,(45±1)℃艾灸组VEGF-A、HIF-1α及p-Akt表达水平较高;干预第10天,(45±1)℃艾灸组VEGF-A及p-Akt表达水平显著高于其他组(均P<0.05)。结论 (45±1)℃艾灸可促进压力性损伤创面愈合,缩短愈合时间,提高修复效果。

    Abstract:

    Objective To observe the effect of different moxibustion temperatures on wound repair in rats with pressure injury. Methods Sixty-five SPF SD male rats were selected and 60 of them were subjected to perform pressure ulcer modeling according to the principle of ischemia-reperfusion. Upon successful modeling, the rats were randomly divided into 1 model group, and 3 moxibustion groups [the (42±1)℃ group, the (45±1)℃ group and the (48±1)℃ group], with 15 rats in each group. The wound of the model group was disinfected with iodophor once a day for 12 days. Moxibustion groups were treated with moxibustion using moxa sticks with a diameter of 18 mm at varied temperatures, 10 min each time, once a day, for 12 days. On the 3rd, 7th and 10th day of intervention, wound healing, healing time, healing rate and pathological changes of the four groups were observed and compared, and the expression levels of vascular endothelial growth factor A (VEGF-A), HIF-1α and p-Akt were observed. Results On the 3rd, 7th and 10th day of intervention, the wound healing time of moxibustion groups was significantly shorter than that of the model group;the wound healing time of the (45±1)℃ group was shorter than that of the (42±1)℃ group and the (48±1)℃ group; the wound healing rate of the (45±1)℃ group was higher than that of the (42±1)℃ group and the (48±1)℃ group (all P<0.05). The results of HE staining showed that the number of inflammatory cells in the (45±1)℃ group were less than the other groups, the new granulation tissue was more than the other groups, and the vascular endothelial cells and fibroblasts were more closely arranged. Immunohistochemical results showed that the expression levels of VEGF-A, HIF-1α and p-Akt in moxibustion groups were higher than those in the model group on the 3rd and 7th day of intervention (all P<0.05). Compared with the (42±1)℃ group and the (48±1)℃ group, the expression levels of VEGF-A, HIF-1α and p-Akt in the (45±1)℃ group were higher (all P<0.05). On the 10th day of intervention, the expression levels of VEGF-A and p-Akt in the (45±1)℃ group were higher than those in the other groups (all P<0.05). Conclusion Moxibustion at (45±1)℃ could promote wound healing, shorten the healing time, and improve repair effect.
