摘要:

综述青少年慢性病患者过渡期护理相关概念、护理内容、评估工具、影响因素及干预措施，为我国青少年慢性疾病患者过渡期护理研究及实践提供参考。

Abstract:

This paper reviews the concept, contents, assessment tools, associated factors and interventions regarding transition care for adolescents with chronic illnesses, so as to provide reference for research and practice of transition of care for adolescents from pediatric services to adult health services in China.
