Objective To explore the effects of regional medical alliance based transitional care on self-care agency and quality of life of stroke patients, and to provide reference for delivery of continuing care for stroke patients. Methods A total of 147 stroke patients discharged from hospital were chronologically divided into a control group of 75 and an intervention group of 72. The control group was given routine follow-ups, while the intervention group received "hospital-community-family" continuing care which was provided by regional medical alliance. Self-care agency and quality of life of patients in both groups were measured at discharge and 6 months after discharge. Results After the intervention, the scores of self-care agency and quality of life in the intervention group were significantly higher than those in the control group (P<0.01 for both). Conclusion The implementation of "hospital-community-family" continuing care by regional medical alliance can improve self-care ability and quality of line of stroke patients.
